| The CalFresh Outreach Coordinator serves as a case coordinator by monitoring incoming referrals for food assistance, overseeing their progression, and completing action plans and/or any follow-up necessary. The Outreach Coordinator responds to referrals, develops and coordinates interventions through appropriate resources, meets and follows-up with students, and tracks student completion of recommendations. They will work closely with campus resource providers, clinicians, counselors, health educators, student affairs units/departments, academic affairs and other departments in coordinating responses that meet the needs of individual students of concern. |
